Your primary care physician is a partner in your wellness, helping to keep you healthy and working with you to manage any medical challenges you may encounter.

At present, 15 Saint Francis-affiliated practices have earned national recognition as Patient-Centered Medical Homes. This means that they meet exacting standards by providing care that is:

  • Comprehensive, addressing every medical need for each patient
  • Patient-centered and relationship-based
  • Coordinated, minimizing the potential for complications
  • Accessible, with shorter waiting times for urgent needs, more in-person physician hours, and around-the -clock access to a member of each healthcare team
  • Safe and of high quality 

Primary care physicians are key to the success of patient-centered medical homes, which greatly minimize the incidence, length, and severity of hospital stays. The result is better health for everyone.

Leading the Way to the Future

The Affordable Care Act places more importance than ever on primary care physicians and Patient-Centered Medical Homes. To help meet these healthcare demands, in 2012 Saint Francis partnered with the University of Connecticut School of Medicine to create the Connecticut Institute for Primary Care Innovation (CIPCI). 

Under the leadership of Crystal Clark, MD, MPH, and the research mentorship of Greg Makoul, PhD, CIPCI will work toward the following outcomes:

  • Improving primary care education for medical students and residents
  • Increasing retention of primary care trainees and physicians
  • Conducting groundbreaking research on the best ways to deliver primary care
  • Providing service to the community of practicing primary care physicians by helping them keep pace with change
