Aetna and Saint Francis Healthcare Partners Reach New Accountable Care Agreement

New accountable care model will serve more than 18,000 members in Greater Hartford 

(Hartford, Conn. October 7, 2014)  — Aetna (NYSE: AET) and Saint Francis HealthCare Partners (SFHCP) today announced a new accountable care agreement that is designed to improve the coordination and delivery of patient care to more than 18,000 Aetna members in Connecticut.   

SFHCP is a joint venture between a community of exceptional physicians and Saint Francis Hospital and Medical Center (SFH&MC). SFHCP is a clinically integrated network which coordinates care and is accountable for cost, quality and patient satisfaction for the health care they provide. SFHCP includes more than 735 primary care physicians and specialists and over 243 physician assistants, nurse practitioners and mid-wives in more than 200 medical practices. 

Aetna members who use SFHCP will experience more coordinated care, particularly those patients with chronic or complex conditions, and enhanced access to appropriate care. The accountable care arrangement also is designed to improve the flow of information to treating physicians at SFHCP.

Under the new agreement, more than 18,000 Aetna commercial health plan members in either self-insured or fully-insured group plans will be served by the ACO. Aetna members served by this new model are ones who primarily received care from SFHCP physicians over the last 24 months, as well as those who seek care from SFHCP physicians following the start of the agreement. While health plan benefits will not change, members will be part of a select group who will receive highly coordinated, personalized care. To support these members, SFHCP coordinators will complement Aetna’s care management programs, in an effort to lower the number of days patients spend hospitalized and improve member health outcomes.  

The careful tracking of quality metrics is a critical part of why ACOs are successful at improving patient care and are fundamental to the incentive structure. This agreement includes a shared savings model that rewards SFHCP physicians for meeting certain quality and efficiency measures such as:  

  • The percentage of Aetna members who receive recommended preventive care and screenings; 
  • Better management of patients with chronic conditions such as diabetes and heart failure; 
  • Reductions in avoidable hospital readmission rates; and 
  • Reductions in unnecessary emergency room visits.  

Aetna is working with health care organizations across the country to develop products and services that support value-driven, patient-centered care for all health care consumers.
