The Principal Investigator is responsible for notifying the IRB, in writing, when the study is closed. A final report must be prepared on the Protocol Progress Report Request for Reapproval/Closure Form which is available for download in Microsoft Word format.

Please submit the original Protocol Progress Report Request Reapproval/Closure form and 11 copies.
