Administering the Consent Agreement

- Give a copy of the form to the patient.
- Read the form aloud as the subject reads it to him/herself.
- After reading it aloud, summarize what is stated on the form.
- Allow time for questions.
- Schedule a break to allow patient time to consider whether or not to participate.
- Once the form is signed and dated, give a copy of consent form to patient.
- Document how consent was administered and amount of time spent during the consent process.
